1 - The Kyshtym disaster was a radioactive contamination accident that occurred on 29 September 1957 at Mayak, a plutonium production site for nuclear weapons and nuclear fuel reprocessing plant in the Soviet Union. It measured as a Level 6 disaster on the International Nuclear Event Scale (INES), making it the third most serious nuclear accident ever recorded, behind the Fukushima Daiichi nuclear disaster and the Chernobyl disaster (both Level 7 on the INES).

2 - The Three Mile Island accident was a partial nuclear meltdown that occurred on March 28, 1979, in reactor number 2 of Three Mile Island Nuclear Generating Station (TMI-2) in Dauphin County, Pennsylvania, United States. It was the most significant accident in U.S. commercial nuclear power plant history. Level 5 on the INES.

3 - A total loss of coolant led to a power excursion and explosion of an experimental nuclear reactor in a large cave at Lucens (21 January 1969 - INES Level: 4 - Lucens, Canton of Vaud, Switzerland). The underground location of this reactor acted like a containment building and prevented any outside contamination. The cavern was heavily contaminated and was sealed. No injuries or fatalities resulted.

4 - The Fukushima Daiichi nuclear disaster (Level 7 on the INES) was an energy accident at the Fukushima I Nuclear Power Plant in Fukushima, initiated primarily by the tsunami following the Tōhoku earthquake on 11 March 2011. Immediately after the earthquake, the active reactors automatically shut down their sustained fission reactions. However, the tsunami destroyed the emergency generators cooling the reactors, causing Reactor 4 to overheat from the decay heat from the fuel rods. The insufficient cooling led to three nuclear meltdowns and the release of radioactive material beginning on 12 March. Several hydrogen-air chemical explosions occurred between 12 March and 15 March.

5 - The Chernobyl disaster (Level 7 on the INES) was a catastrophic nuclear accident that occurred on 26 April 1986 in the No.4 light water graphite moderated reactor at the Chernobyl Nuclear Power Plant near Pripyat, in what was then part of the Ukrainian Soviet Socialist Republic of the Soviet Union (USSR).
